RESUMO

Objective:To investigate the efficacy of floating acupuncture therapy in the treatment of chronic refractory pressure ulcers and its effects on serum levels of superoxide dismutase, malondialdehyde and insulin-like growth factor-1.Methods:A total of 60 patients with chronic refractory pressure ulcers who received treatment in The Second Hospital of Jiaxing from October 2019 to October 2021 were included in this study. They were randomly divided into observation and control groups, with 30 patients in each group. The patients in the control group were given routine treatment, while the patients in the observation group were given floating acupuncture therapy based on routine treatment. Clinical efficacy, wound healing rate after 7, 14 and 21 days of treatment, the time of emerging granulation tissue, the time of shedding rotten meat, and the time of wound healing, changes in serum levels of superoxide dismutase, malondialdehyde and insulin-like growth factor-1 after 21 days of treatment relative to those before treatment were compared between the two groups.Results:Total response rate in the observation group was significantly higher than that in the control group [90.0% (27/30) vs. 66.7% (20/30), χ2 = 4.81, P < 0.05]. Wound healing rate after 7, 14 and 21 days of treatment in the observation group was significantly higher than that in the control group ( t = -5.45, -7.77, -7.51, all P < 0.001). The time of emerging granulation tissue, the time of shedding rotten meat, and the time of wound healing in the observation group were significantly shorter than those in the control group ( t = 5.69, 9.26, 8.91, all P < 0.001). After 21 days of treatment, serum levels of superoxide dismutase, malondialdehyde and insulin-like growth factor-1 in the observation group were (96.52 ± 10.43) U/mL, (4.32 ± 0.78) μmol/L, (43.52 ± 8.29) μg/L, respectively, which were significantly different from (83.76 ± 6.81) U/mL, (5.48 ± 0.92) μmol/L, (36.78 ± 9.18) ng/mL, in the control group ( t = 5.61, 5.27, 2.98, all P < 0.05). Conclusion:Floating acupuncture therapy is effective in the treatment of refractory pressure ulcer. It can accelerate wound healing, increase serum superoxide dismutase and insulin-like growth factor-1 levels, and decrease serum malondialdehyde level.

Objective@#To investigate the expression of discoid domain receptor 2 (DDR2) and interferon-induced transmembrane protein 1 (IFITM1) in nasopharyngeal carcinoma (NPC) patients and their clinical significance.@*Methods@#From November 2014 to November 2015, 65 patients with nasopharyngeal carcinoma were selected. All patients underwent nasopharyngeal biopsy. All specimens were embedded in paraffin after biopsy. All specimens were confirmed by pathology. Another 65 rhinitis mucosa specimens with chronic inflammation confirmed by pathology in the same period were selected as control group. The clinical and pathological data of the two groups were collected. The mRNA and protein expression levels of DDR2 and IFITM1 in nasopharyngeal carcinoma tissues were detected by quantitative real-time polymerase chain reaction (qRT-PCR) and immunohistochemistry. The correlation between them was analyzed by Pearson. All patients were followed up for 3 years, and the 3-year survival period of the patients was analyzed by Kaplan-Meier method.@*Results@#The results of qRT-PCR showed that the relative expression levels of DDR2 and IFITM1 in nasopharyngeal carcinoma tissues were significantly higher than those in control group (P<0.05). Immunohistochemical results showed that the positive rates of DDR2 and IFITM1 in nasopharyngeal carcinoma were significantly higher than those in control group (P<0.05). The results of expressions of DDR2 and IFITM1 and the clinicopathological analysis of the patients showed that the expression of DDR2 was correlated with TNM stage, differentiation degree and infiltration depth, and IFITM1 was correlated with lymph node metastasis, TNM stage, differentiation degree and infiltration depth (P<0.05). Pearson correlation analysis showed that there was correlation between DDR2 and IFITM1 (r=0.608, P<0.01). Kaplan-Meier analysis showed that the progression-free survival rate (41.17%) and total survival rate (52.94%) in DDR2 negative expression group were significantly higher than those in positive expression group (16.67%, 18.75%, P<0.05). The progression-free survival rate (42.86%) and total survival rate (61.90%) in IFITM1 negative expression group were significantly higher than those in positive expression group (18.18%, 20.45%, P<0.05).@*Conclusions@#DDR2 and IFITM1 are highly expressed in nasopharyngeal carcinoma, which may be involved in the occurrence and development of nasopharyngeal carcinoma and significantly affect the prognosis of patients.

@#Objective To explore the effect of rehabilitation training combined with rational-emotive therapy on post-stroke depression (PSD).Methods 51 PSD patients were randomly divided into the treatment group (n=25) and control group (n=26). All patients received routine rehabilitation training, simultaneously took 20 mg of fluoxetine hydrochloride dispersible tablets per day. Based on above, the treatment group was given physiological rehabilitation treatment with rational-emotive therapy in early stage, 30 min each time, continuously 6 weeks. The two groups were assessed with Zung Self-rating Depression Scale and Fugl-Meyer Motor Scale before and after treatment.Results Afer treatment, the scoers of Zung Self-rating Depression Scale and Fugl-Meyer Motor Scale of the treatment group was higher than that of the control group ( P<0.05).Conclusion The rational-emotive therapy might improve the depression condition of post-stroke, enhance patients' motor function.

Objective To observe the effect of early occupational therapy intervention on unilateral spatial neglect caused by stroke.Methods A total of 92 patients with unilateral spatial neglect caused by stroke were ran- domly divided into a treatment group(46 cases)and a control group(46 cases).The cases of the control group were treated by routine clinical treatment plus positioning on the bed,passive joint motion,bridging movement,facilitation techniques,balance training in sitting and standing positions,gait training immediately after the patients' conditions got stable.The patients in the treatment group were treated with activities of daily living(ADL)training in addition to the interventions employed in the control group.Results Effect of the treatment group are superior to that of the control group as reflected by the scores with Fug1-Meyer scale and Barthel Index(P

Objective To assess the influence of changes in alveolar ventilation on gastro-intestinal perfusion measured via a nasogastric tonometer.Methods Sixteen ASA Ⅰ - Ⅱ patients ( 7 male, 9 female) aged 15-67 yr, undergoing elective intracranial operation were included in this study. The patients were premedicated with oral ranitidine 150 mg and intramuscular phenobarbital 0.1 g and atropine 0.5 mg. Anesthesia was induced with midazolam 1-2 mg, fentanyl 2 ?g? kg-1 , propofol 1.5-2.0 mg ? kg-1 and vecuronium 0.1 mg ? kg-1 , and maintained with 0.5%-2.0% isoflurane inhalation and intermittent iv boluses of fentanyl and vecuronium. The patients were intubated and mechanically ventilated after induction of anesthesia. The ventilatory settings were : RR 10 bpm; FGF 1 L? min-1; I: E = 1:2; the initial VT was 7 ml? kg-1 (T1 ) which was gradually increased to 8 ml ?kg-1(T2), 9ml?kg-1(T3) and 10 ml?kg-1 (T4) and then returned to 7 ml ? kg-1 again. Each VT was maintained for 1 h. A nasogastric tonometer (Tonocap, Datex-Ohmeda, Finland) was inserted into stomach and automatically measured gastric intramucosal CO2 tension (PgCO2) every 10 min. Radial artery was cannulated for direct MAP monitoring and blood sampling. Blood gas analysis was performed every hour at various VT levels. RESUMO

Objective Controlled hypotension induced by different drugs may have different effects on splanchnic perfusion. The purpose of this study was to assess the effects of nicardipine- induced hypotension on splanchnic perfusion. Methods Twenty-three ASA Ⅰ-Ⅱ patients (14 male, 9 female) aged 21-60 yr undergoing meningioma resection were studied. The patients were premedicated with ranitidine 150 mg per os and atropine 0.5 mg i.m. Anesthesia was induced with propofol 1.5-2.0 mg?kg-1 , fentany1 2.0?g?kg-1 and vecuronium 0.1 mg? kg-1 and maintained with isoflurane inhalation and intermittent i. v. boluses of fentanyl and vecuronium. The patients were mechanically ventilated after tracheal intubation and PETCO2 was maintained at 33-35 mm Hg. Nicardipine-induced hypotension started from opening of mininges to the resection of tumor. 0.01% nicardipine infusion was started at 30 ml?h-1 and adjusted to reduce systolic blood pressure by 30% of the baseline or MAP to≥60 mm Hg. The adequacy of splanchnic perfusion was defined by gastric intramucosal CO2 tension (PgCO2), the intramucosal pH (pHi) and the difference between intramucosal and arterial PCO2(PCO2 gap) and was assessed before induction of anesthesia, the 1st, 2nd and 3rd hour during controlled hypotension and 1, 2 hour after termination of controlled hypotension. Results The MAP was maintained at about 62 mm Hg during controlled hypotension. The heart rate was stable. Hct was significantly decreased at 1st, 2nd and 3rd hour of hypotension compared with baseline but the mean Hct was greater than 30% . The intramucosal pH decreased slightly with no statistical significance and pHa, PaCO2 , PgCO2 and PCO2 gap were almost unchanged during controlled hypotension compared with the baseline values. Conclusion Nicardipine-induced hypotension is safe when MAP is maintained at≥60 mm Hg and has no adverse effects on splanchnic perfusion.

Objective To clone and sequence cDNA encoding 3-hydroxy-3-methylglutaryl-coenzyme A reductase(HMGR) from Atractylodes lancea.Methods The cDNA,encoding HMGR in A.lancea,was amplified by RACE strategy with the cDNA of the total RNA of young leaves as the template.The partial fragments of HMGR were cloned and sequenced.Results The analysis results revealed that the conserved fragments were 458 bp.At the same time,the two fragments had been obtained 84.28% identification in nucleotide acid and 92.11% identification in corresponding amino acid,named as HMGRcr1 and HMGRcr2,respectively.It was deduced that they may be members of the HMGR gene family in A.lancea.Sequencing analysis showed that HMGRcr1 and HMGRcr2 had high identity with HMGR from other plants.Conclusion The cDNA encoding HMGR from A.lancea is cloned and reported for the first time.The work will provided a foundation for exploring the mechanism of terpenes biosynthesis and application to the other medicinal plants.
